Motor Insurance

Documents required for processing a claim:

Motor Accident Repair Claims

  • Please click here for full listing of our Authorised Partner Repairer.
  • Accident Report Form.
  • Police Report.
  • Copy of the Vehicle’s Registration Card (front and back).
  • Copy of the insured’s and driver’s driving licence and identity card.
  • Requirement of Photographs Before Towing
    • Take photographs at the scene of the accident of all vehicles involved with the number plates and severity of damages to the vehicles (before towing). Photos must show both the direction of the road with the demarcation line of the road. You are also requested to take photographs of injured passengers (third party vehicles).
    • To provide explanation letter if no photographs are taken at the scene of accident.

Motor Windscreen Damage Claim

  • If windscreen is replaced and claim submitted through our Authorised Windscreen Repairer.
    • Police report is waived
    • No payment to workshop required

Documents required are as follows: –

  • Complete and sign the Windscreen Claim Form. Claim Form can be printed from Merimen Online if claim is submitted through our Authorised Windscreen Repairer
  • Copy of the Vehicle’s Registration Card (front and back)
  • Copy of the insured’s and driver’s NRIC and Driving Licence
  • Copy of the Policy Schedule
  • Please provide documentary proof of tinted film if there is a claim for the tinted film
  • If a claim is submitted through Non Authorised Windscreen Repairer

Kindly submit the following documents:

  • Complete and sign the Windscreen Claim Form
  • Original Police report
  • Original bill for the replacement of the windscreen / window glass
  • Copy of the Vehicle’s Registration Card (front and back)
  • Copy of the insured’s and driver’s NRIC and Driving Licence
  • Copy of the Policy Schedule
  • Please provide documentary proof of tinted film if there is a claim for the tinted film
  • Photographs showing the
    • General view of the vehicle with the damaged windscreen / window glass (four angle views (front and back) with visible number plate)
    • Close-up of the damaged windscreen / window glass
    • The damaged windscreen must be placed in front of the vehicle number plate
    • Before and after replacement of the logo / manufacturer of the glass

Motor Theft Claim

  • A complete Motor Theft Report Form.
  • Copy of Vehicle’s Registration Card.
  • Original certification of Insurance.
  • Police Report.
  • Hire Purchase Agreement (if applicable).
  • Hire Purchase Release Letter / Surat Pengesahan e-Hakmilik JPJ-FIS (if applicable).
  • Copy of insured’s and driver’s identity card and driving licence.
  • Copy of last Service Bill.
  • If the Insured is a corporate body:
    • Copy of the certificate of incorporation of the company or Registration of Business.

Motor Theft Recovered Claim

In the event your motor vehicle was stolen and subsequently recovered before your claim is settled, please contact us immediately. Our appointed Loss Adjuster shall inspect your motor vehicle at the police station, before it is removed to our Company Authorised Partner Repairer selected by you from our list. Failure to act/comply on the above, would be a breach of this condition and we shall have the right to decline liability under Section A of motor insurance policy. Please Remember:

  • Notify us immediately if a third party submits a claim against you or your authorised driver.
  • Do not negotiate or admit liability to the claim made by the third party without our consent.

  • Notify us immediately of the incident or loss by calling our Customer Service Centre at +603 2034 9888 or our nearest branch.
  • We will advise you of the claim requirements upon receipt of your notification, including assigning an adjuster to assess your loss, if necessary.
